Background and purpose: The incidence of brachial plexus palsy (BPP) has decreased recently, but the indivi-d-ual's quality of life is endangered. To provide better chan-ces to BPP neonates and infants, the Department of Developmental Neurology worked out, introduced, and applied a complex early therapy, including nerve point stimulation. Methods: After diagnosing the severity of BPP, early intensive and complex therapy should be started. Appro-x-imately after a week or ten days following birth, the slightest form (neurapraxia) normalizes without any intervention, and signs of recovery can be detected around this period. The therapy includes the unipolar nerve point electro-stimulation and the regular application of those elemen-tary sensorimotor patterns, which activate both extremities simultaneously. Results: With the guideline worked out and applied in the Department of Developmental Neurology, full recovery can be achieved in 50% of the patients, and even in the most severe cases (nerve root lesion), functional upper limb usage can be detected with typically developing body-scheme. Conclusion: Immediately starting complex treatment based on early diagnosis alters the outcome of BPP, providing recovery in the majority of cases and enhancing the everyday arm function of those who only partially benefit from the early treatment.

Brachial plexus (BP) injury during labor is called obstetric brachial palsy (OBP). It is an abnormality that occurs in the upper extremity of the body due to excessive stretching of the neural roots of the BP. Every motor skill that the child with OPB acquires will be hampered by the deficiency in the movement of an upper limb (MS), impacting his motor experimentation. To modify their motor behavior, task-directed therapy can contribute to the function of the affected upper limb, because it is characterized by a protocol of functional exercises, which is still scarce in researches aimed at this morbidity. The objective of the study was to evaluate the effect of a motor intervention by means of five directed tasks on the manual skills of the upper limb of a child with OBP, as well as on his gross motor function. The child in the study was 17 months old, with left OBP, with weakness of shoulder abduction, external rotation, elbow flexion and wrist drop. He underwent 24 physiotherapy sessions with directed tasks for 45 minutes, three times a week. The Manual Ability Classification System (MACS) was used to classify the degree of severity of the MSE. The Gross Motor Function Measure (GMFM-66) quantitatively measured motor and static aspects and the Gross Motor Function Classification System (GMFCS) was used to determine which level best represented the abilities and limitations in the child's gross motor function. The targeted tasks were based on the model of the Induced Constraint Therapy (ICT) protocol, being: find the surprise; orange spoon; hair elastic fitting; fishing for bottle caps; stacking blocks. In the post-intervention assessments, the MACS showed improvement in the tasks of finding the surprise, orange spoon, and fishing for lids, but remained the same in the level of the activities of fitting the rubber bands and stacking the blocks. The GMFM-66 obtained an increase in the final score and the GMFCS remained at the level I. The GMFM-66 showed a 4.99% improvement at the end of the intervention. The activities finding the surprise, picking oranges, and fishing for bottle caps showed an improvement in the MACS level classification compared to the initial assessment. The findings show better muscle recruitment, with refinement in elbow flexion movements, forearm supination and external rotation (ER) of the shoulder (AU)


A lesão do plexo braquial (PB) durante o parto é denominada paralisia braquial obstétrica (PBO). É uma anormalidade que ocorre na extremidade superior do corpo, em consequência de um estiramento excessivo das raízes neurais do PB. Toda habilidade motora que a criança com PBO adquirir será dificultada pela deficiência na movimentação de um membro superior (MS), repercutindo em suas experimentações motoras. Para modificar seu comportamento motor, a terapia por tarefas direcionadas pode contribuir na função do MS afetado, pois a mesma caracteriza-se com um protocolo de exercícios funcionais, que ainda há escassez em pesquisas voltados a esta morbidade. O objetivo do estudo foi avaliar o efeito de uma intervenção motora através de cinco tarefas direcionadas sobre as habilidades manuais do membro superior de uma criança com PBO, bem como sua função motora grossa. A criança do estudo tinha 17 meses, com PBO à esquerda, com fraqueza de abdução de ombro, rotação externa, flexão de cotovelo e queda do punho. Realizou 24 sessões de fisioterapia com tarefas direcionadas por 45 minutos, três vezes por semana. Para classificar o grau de severidade do MSE, foi utilizado o Manual Ability Classification System (MACS). A Medida da Função Motora Grossa (GMFM-66) mediu quantitativamente aspectos motores e estáticos e o Sistema de Classificação da Função Motora Grossa (GMFCS) foi utilizado para determinar qual nível melhor representou as habilidades e limitações na função motora grossa da criança. As tarefas direcionadas foram baseadas no modelo do protocolo de terapia por contensão induzida (TCI), sendo: achar a surpresa; colher laranja; encaixar elástico de cabelo; pescaria de tampinhas; empilhar blocos. Nas avaliações pós-intervenção, o MACS demonstrou melhora no nível das tarefas de achar a surpresa, colher laranja e pescaria de tampinhas, mas manteve-se igual no nível das atividades de encaixar os elásticos e empilhar os blocos. O GMFM-66 obteve aumento do escore final e o GMFCS manteve-se no nível I. O GMFM-66 apresentou melhora de 4,99% ao final da intervenção. As atividades de achar a surpresa; colher laranjas e pescaria de tampinhas obtiveram melhora na classificação no nível do MACS quando comparadas à avaliação inicial. Os achados evidenciam melhor recrutamento muscular, com refinamento nos movimentos de flexão de cotovelo; supinação do antebraço e rotação externa (RE) de ombro (AU)

INTRODUCTION: obstetrical brachial plexus palsy is a relatively rare condition that has not disappeared although important progress has been made in obstetrics. The purpose of this study is to analyze the epidemiological, clinical, therapeutic and evolutionary features of this disorder in our context. METHODS: we conducted a retrospective study of the medical records of newborns with obstetrical brachial plexus palsy treated at the Bouaké University Hospital over a period of two years. Children treated after the age of 3 months weren´t included. The variables studied were the epidemiological, therapeutic and evolutionary features. RESULTS: the study included 60 patients, reflecting a rate of 28.5%. There were 31 (52%) girls. The average age of patients was 8 days (D0 and D35]. Multiparous mothers accounted for 94% of cases. Birth took place in a health center in 97% of cases. All children were born at term, 57 (95%) in cephalic presentation. Delivery was normal in 74% of cases. Average birthweight was 3604g [2150g and 4500g]. Forty seven cases (78%) had C5-C5-C6 palsies . Immobilization elbow off body associated with rehabilitation was performed in 51 children (85%). Rehabilitation was performed immediately in 9 children (15%). Functional recovery of the injured limb was complete in 50 children (83%) after a follow-up period of 6 months. CONCLUSION: obstetrical brachial plexus palsy is a current obstetric disorder. Conservative treatment, that is the only alternative in our context, gives good results.

INTRODUCCIÓN: La parálisis braquial obstétrica cursa con afectación del miembro superior y da lugar a secuelas estructurales y de la función permanentes que limitan el rango articular de movimiento y reducen la independencia en las actividades cotidianas, así como la participación del niño dentro de su entorno natural. OBJETIVO: Describir los tratamientos de fisioterapia empleados en la actualidad para incrementar la funcionalidad de la extremidad superior afectada en niños de 0 a 10 años diagnosticados de parálisis braquial obstétrica. PACIENTES Y MÉTODOS: Se realizó una búsqueda bibliográfica de los trabajos publicados entre 2009 y 2018 en las bases de datos PubMed, PEDro, ScienceDirect y Cochrane Library. RESULTADOS: Tras aplicar los criterios de inclusión/exclusión, se obtuvieron diez estudios con resultados favorables para la funcionalidad de la extremidad superior afectada y la densidad de mineralización ósea, independientemente de la técnica de elección utilizada durante la intervención. Se analizaron los programas de fisioterapia empleados en los diferentes estudios, como la terapia de movimiento inducido por restricción, el kinesiotape, la electroterapia, la realidad virtual y el uso de férulas u ortesis. CONCLUSIÓN: Todas las técnicas descritas sugieren resultados favorables para la funcionalidad del miembro superior en niños de 0 a 10 años con parálisis braquial obstétrica


INTRODUCTION: Obstetric brachial palsy is involved with affected upper limb, resulting in permanent structural and function sequelae that limit the motion articular range and reduce independence in daily activities, as well as the children participation in their natural environment. AIM: To describe the physiotherapy treatments currently used to increase the affected upper limb functionality in children from 0 to 10 years diagnosed with obstetric brachial palsy. PATIENTS AND METHODS: A bibliographic search of published studies between 2009 and 2018 was carried out in the PubMed, PEDro, ScienceDirect and The Cochrane Library databases. RESULTS: After applying the inclusion/exclusion criteria, ten studies were obtained with favorable results for the affected upper limb functionality and bone mineralization density, regardless of the technique of choice used during the intervention. The used physiotherapy programs in the different studies such as Constraint Induced movement therapy, kinesiotape, electrotherapy, virtual reality and use of splints or orthotics were analyzed. CONCLUSION: All the described techniques suggest favorable results for the affected upper limb functionality in obstetric brachial palsy from 0 to 10 years

RESUMO

Neonatal brachial plexus palsy is a complex disorder that requires evaluation of the primary nerve injury and associated sequelae. There is no standardized approach to management, and many treatment options are available. We present a case of Erb palsy with a unique multidisciplinary approach to nonsurgical and surgical management. Inherent in the case is the complexity of decision-making, and we review the literature on treatment options and the rationale for the approach, which resulted in near normal functional recovery.
